| 内容記述 | UHC達成国であるタイの医療保障3制度、特に公務員等を対象としたCSMBSと一般国民向けのUCSの不均衡に関し、給付内容と支払方式、運営機構に係る差異の現状を概観した。次いで是正への取組みと、CSMBSの取組みを違法とする判例、及び救急医療の運営面の改善について整理した。次いで改革が期待通り進まない背景の仮説として、国民の納得と医療消費の階層化の可能性、UCSの給付水準の上昇による不満の緩和、医療機関等有力な反対者の存在の3点を提示した。 その上で、同様に分立した制度の下でUHCを達成する日本の制度での制度間格差の問題の経緯から明らかとなるタイの改革の困難さの理由として、CSMBSが退職者も加入する仕組みであること、CSMBSとUCSが税方式であること、加入者が費用単価の不均衡を実感しにくいこと、一定の医療消費の階層化が存在するであろうこと、所管庁が別々なことを挙げ、制度改革を可能にする政治、経済、社会環境が整う必要性を示唆した。 The author first reviewed the disparity of benefit contents, payment mechanisms and management organization of the three schemes for health protection in Thailand that achieved UHC, especially CSMBS for the civil servants and UCS for general public. Next, the author reviewed the efforts for adjustment by Thai authority, a judicial precedent that judged a CSMBS policy as unconstitutional, and the practical improvement in emergency medicine. Then the author pointed out three backgrounds that might hinder the reform as expected, namely, the possibility of the understanding of the public and stratification of medical consumption, ease of public dissatisfaction by the improvement of benefit level of UCS, and the influential dissenters such as medical institutions. Then, by comparing it with the history of issues of disparity in the Japanese UHC with multiple schemes, the author suggested 1) the fact that CSMBS continues the coverage after retirement, 2) both CSMBS and UCS are tax-financed, 3) the disparity may be unnoticeable to the enrollees, 4) medical consumption may be stratified, as well as 5) different agencies are supervising each scheme are the reasons of difficulty for Thai, and pointed out that political, economic, and social environment need to be matured to enable the reform. |
